ABSTRACT

OBJECTIVE@#To explore the discrepancy between behavioral-indicated candidacy and perceived candidacy (behavioral-perceived gap) and its associated factors of human immunodeficiency virus (HIV) pre-exposure prophylaxis (PrEP) among men who have sex with men (MSM), so as to identify the focus population of PrEP interventions and to design and implement targeted interventions.@*METHODS@#We recruited a sample of 622 HIV-negative MSM who were regular clients of a community-based organization located in Chengdu, China, from November to December 2021. A cross-sectional questionnaire was used to collect the participants' information on social demographics, PrEP-related knowledge and cognitions, and risk behaviors. In this study, behaviorally eligible for PrEP was defined as performing at least one type of high-risk behavior in the past six months, including inconsistent condom use, sex with an HIV-positive partner, confirmed sexual transmitted infections (STI) diagnosis, substance use, and post-exposure prophylaxis (PEP) experience. Logistic regression models were fitted, and multivariate analyses were adjusted for social demographics.@*RESULTS@#Among the 622 eligible participants, 52.6% (327/622) were classified as behaviorally eligible for PrEP. Only 37.9% (124/327) of the participants perceived themselves as appropriate candidates for PrEP and 62.1% (203/207) had discrepancy between behavioral-indicated and perceived candidacy. 85.9% (281/327) had heard of PrEP, and 14.2% (40/281) accessed PrEP information through health care providers. Of the 327 participants eligible for behavior-indicated PrEP use, about half (47.1%) knew how to obtain PrEP medication and 33.0% had a professional PrEP counseling experience. The majority (93.3%) had no or few friends using PrEP. 54.1% scored eight or above in PrEP knowledge level. 66.7% reported having two or more sexual partners in the past six months. After adjusting for age and recruitment channel, we found six factors that were associated with perceived candidacy for PrEP, including PEP use [adjusted odds ratio (ORA)=2.20; 95% confidence interval (CI): 1.33-3.63], PrEP availability (ORA=1.69; 95%CI: 1.06-2.68), a greater number of PrEP-using friends (ORA=4.92; 95%CI: 1.77-13.65), PrEP know-ledge (ORA=2.21; 95%CI: 1.38-3.56), multiple sexual partnership (ORA=1.77; 95%CI: 1.07-2.94), and perceiving a higher risk of HIV infection (ORA=4.02; 95%CI: 1.73-9.32). Substance use during sex and PrEP information channel were not statistically associated with this beha-vioral-perceived gap.@*CONCLUSION@#We observed a high discrepancy between behavioral-indicated and perceived candidacy for PrEP among Chengdu MSM in China. Future PrEP implementation efforts should be made in skills training in assessing HIV infection risk, increasing PrEP knowledge, providing professional PrEP counselling, and fostering PrEP support environment.

ABSTRACT

ABSTRACT

ABSTRACT Objective: Although the development of prevention and treatment strategies for sexually transmitted infections in key groups has improved over the years, they still remain a challenge for health systems worldwide. In this context, the objective of this study is to assess the seroprevalence in the tested population, with an emphasis on key populations, aiming at identifying the participants' profile and consequently the development of testing strategies. Methods: The present study analyzed the seroprevalence of HIV, syphilis, and hepatitis B and C, and the epidemiological profiles of key and general populations tested at a reference public health facility for sexually transmitted infections testing and counseling in the city of Curitiba, Southern Brazil. A cross-sectional study was conducted to report data from 2010 to 2019. Results: A total of 67,448 samples were analyzed, 9,086 of these tested positive, 3,633 (56%) for HIV, 4,978 (77%) for syphilis, 340 (5%) for hepatitis C virus (HCV), and 135 (2%) for hepatitis B virus (HBV). Overall, most of the participants were men (79 to 87%), and predominantly white. For HIV and syphilis, the predominant age groups were 21-30 years old (48 and 50%), HBV 21-40 years old (31%), and HCV 41-60 years old (25%). A high seroprevalence of HIV and syphilis was observed in the investigated key populations with a higher frequency in sex workers, men who have sex with men, and transgender. Conclusion: The progressive increase in syphilis cases emphasizes the need for effective interventions to enhance adherence to the use of condoms, and to expand diagnosis and treatment for these key populations.

ABSTRACT

ABSTRACT More than half of new HIV infections in 2018 were among key populations, including gay men and other Men who have Sex with Men (MSM). Stigma can have several behavioral consequences, adversely reflecting on the lives of individuals and can be translated into barriers to access to public health services. This paper aimed to analyze the main barriers to accessing HIV testing at a Curitiba Testing and Counseling Center and the linkage of gay men and other MSM with the health service for early HIV treatment. Primary data were collected following the qualitative method, triangulating field observations, document analysis, and interviews. The results pointed to the lack of trained professionals, difficulty receiving health services, acceptance of sexuality, fear of results, and lack of information as the main barriers to testing. Difficulty in accepting the diagnosis, prejudice, and discrimination were some barriers identified to start treatment. We can conclude that the expansion of access to guarantee the rights of those who have more difficulty entering the Unified Health System (SUS) is still a significant challenge.

ABSTRACT

Objective@#To examine the prevalence and associated factors of seeking casual male sexual partners among men who have sex with men (MSM) students in Guangzhou, and to provide a reference for health education and intervention.@*Methods@#A cross-sectional study was conducted to recruit 189 MSM students aged 18 and 28 via voluntary counseling and testing (VCT) clinics, Lingnan partners and Zhitong charity from May 2017 to April 2018. An anonymous electronic questionnaire was administrated to collect information on sociodemographic and sexual behavior characteristics. Chi-square test and non-conditional Logistic regression analysis were performed to explore the associated factors of casual homosexual behavior among MSM students.@*Results@#In the past 6 months, 63.49% (120/189) of them reported casual male partners, and the rate of consistent condom use was 61.67%(74/120). Multivariate Logistic regression analysis showed that participants who had multiple sex partners in the past 6 months were more likely to seek male casual partners(aOR=9.13, 95%CI=4.27-19.53). Individuals who used recreational drugs before sex were at a higher risk of having casual sex behaviors(aOR=3.06, 95%CI=1.08-8.62). MSM students who had steady homosexual partners had a lower risk to seek casual sex(aOR=0.43, 95%CI=0.19-0.98).@*Conclusion@#The proportion of seeking casual male sexual partners among MSM students in Guangzhou was relatively high. The publicity and education should be strengthened among those who had multiple sex partners, with no steady homosexual partners and used recreational drugs before sex, to reduce the casual sex behaviors.

ABSTRACT

Abstract Introduction Homosexuality represents a deviation from the social norms established by a sectorial society which imposes having a mandatory heterosexual pattern. Because of this, having a different sexual orientation implies facing prejudices which have diverse impacts on the family environment. Objective To know the response of the family environment towards the revelation of the sexual orientation from the perspective of homosexual males. Methods This is a qualitative and descriptive study with theoretical sampling and key informing, which used semi-structured interviews and observations on five homosexual males from the city of Puebla, Mexico, whose ages ranged from 19 to 36 years old. Results: The findings clustered around a central category: the family response towards having a homosexual member, from which three subcategories emerged: family beliefs, family conflict, and the loss of the heterosexual normativity, reflected by multiple codes classified in the five stages of the process of a loss proposed by Kübler-Ross. Discussion: The family responses towards homosexuality are dynamic and framed by a denial process which eventually can lead to the acceptance. Conclusions: The family responses towards having a member being homosexual are multiple; thus, an understanding of these responses can help nurses and other health professionals to plan activities and strategies aimed at addressing the process of the family loss of the heterosexual normativity.

ABSTRACT

Objective@#To analysis the status of HIV infection and the related factors among students of men who have sex with men (MSM) from voluntary counseling and testing (VCT) clinics in Changzhoucity Jiangsu province.@*Methods@#A total of 236 subjects with previous male sexual history, 16-25 years of age and less than 3 months of confirmation time of HIV positive infection were recruited in Changzhou from January 2014 to December 2017. Questionnaires were conducted and plasma samples were collected for selenium and HIV antibody testing. The relevant factors of HIV infection among MSM were screened by classification tree model. The model was evaluated using cross validation and receiver operating characteristic (ROC) curve.@*Results@#The age of subjects was (20.76±1.97) years old. The age of the first sex with men was (19.14±1.85) years old and the plasma selenium content was (82.59±11.99) ng/ml. Of the 236 subjects, 74.58% (176 cases) were college students or undergraduates; 8.90% (21 cases) were diagnosed with venereal diseases in the last year; 80.93% (191 cases) received health services in the last year, and the positive rate of HIV antibody was 25.00% (59 cases). Four relevant factors were screened by the classification tree model, including the diagnosis of sexual transmitted diseases (STDs) in the last year, health services in the last year, plasma selenium level and education level. The most important factor was whether STDs were diagnosed in the last year. The estimate of re-substitution and cross-validation of the classification tree model was 0.186 and 0.195, and the standard error was 0.025 and 0.026, respectively. The area under the ROC was 0.706 (P<0.001).@*Conclusion@#The MSM HIV antibody positive rate of VCT students in Changzhou City, Jiangsu Province was 25.0%. The diagnosis of STDs in the last year, receiving health services in the last year, plasma selenium level and education level were relevant factors of HIV infection.

ABSTRACT

Objective@#To study the prevalence of rush poppers use among men who have sex with men (MSM) in Urumqi and to analyze the mediating effect of high-risk sexual behavior between the use of rush poppers and human papillomavirus (HPV) infection.@*Methods@#From January to June 2018, 651 MSM were recruited through network and publicity. Data was collected by using online questionnaire and anal swab were collected from them for HPV genotyping. According to Baron and Kenny's criterion, multivariate logistic regression models was used to analyze the mediation effect of high-risks sexual behaviors (multiple sexual partners, anal intercourse condom use) between rush poppers use and HPV infection.@*Results@#A total of 651 subjects were (32.0±8.0) years old, and 174 subjects (26.7%) had ever used rush poppers in the past 6 months, while 350 subjects (46.9%) had more than 2 sexual partners and 188 subjects (28.9%) did not use anal condom. After adjusting age and marital status, rush poppers use was associated with HPV infection (OR (95%CI) was 2.34 (1.63-3.36), P<0.05), and sexual partners in the past 6 months (OR (95%CI) was 2.72 (1.89-3.93), P<0.05).After adjustment for age, marital status, sexual partners in the past 6 months, and anal condom use, rush poppers use was still associated with HPV infection (OR (95%CI) was 2.21 (1.53-3.19), P<0.05).After adjustment and adjustment of age, marital status, rush poppers use and anal sex condom use, the number of sexual partners at nearly 6 months was still associated with HPV infection (OR (95%CI) was 1.46 (1.05-2.04), P<0.05).@*Conclusion@#Sexual partners in the past 6 months have a mediation effect between rush poppers use and HPV infection in Urumqi. For the prevention of sexually transmitted diseases, we should focus on rush poppers use.
